When is a USMCA certificate of origin form needed?

Products exported to Canada or Mexico that originate from the U.S., Canada or Mexico may be eligible for preferential tariff rates.

A USMCA form is not required for imports if a good(s) is valued at less than $1000 USD. Otherwise, a note accompanying a commercial invoice stating the shipment qualifies as originating goods under the USMCA rules of origin must be be included.

If needed, the USMCA form can be completed and accompany the shipment or be provided to the importer prior to the shipment arriving at customs.

How to complete a USMCA form:

Certifier Name & Address and Tax ID No.: Full Legal name and address, including country, and tax identification number of the certifier.

Exporter Name & Address and Tax ID No.: Full Legal name and address, including country, and tax identification number of the exporter.

Blanket Period: Provide a starting and ending date if multiple shipments of identical goods listed on the commercial invoice will be imported within this period, up to one year.

Producer Name & Address and Tax ID No.: Full Legal name and address, including country, and tax identification number of the producer. If the producer and exporter are the same, write "SAME." If goods from more than one producer are include in a shipment, attach a cross-referenced list of additional producers. It is acceptable to writhe "UNKNOWN" or "Available to CBP ups request" if confidentiality is desired.

Importer Name & Address and Tax ID No.: Full Legal name and address, including country, and tax identification number of the importer. It is acceptable to write "UNKNOWN" or "VARIOUS."

Description of Goods: Fully describe each good as it relates to the invoice description and HS description of the good.

H.S. No.: Provide the H.S. tariff classification to six digits for each good. Learn about the Harmonized System and find your HS No.

Origin Criterion: Determine Origin Criteria (A through D) to be entitled to preferential tariff treatment

Certification Indicator: Specify Certifier's authority (in accordance with Article 5.2) for Certification of the good (A, B, C, or D) based on the following. SELECT ONLY ONE:

A ‐ Certifier is not the Producer, but has a certification or statement from the Producer B ‐ Certifier is the Importer C ‐ Certifier is the Exporter D ‐ Certifier is the Producer, Any good for which you are the Producer, always select Indicator D even if another indicator may apply

Method of Qualification: For each good described in Field #5, where the good is subject to a Regional Value Content (RVC) requirement, indicate "NC" if the RVC was calculated according to the Net Cost method and "TV" if the good was calculated according to the Transaction Value method. If the good was qualified via a tariff shift, indicate "TS." If Origin Criterion A or C, enter "NO."

Country of Origin: Identify country goods originated from.

Form must be signed and dated by the exporter or producer completing the form.
